TheMetropolitan Jewish Health SystemHospice and Palliative Care Leadership nominated Stacey Fanelli for the United Hospital Fund Excellence in Health Care recognition. With more than 20 years of health care experience—and nearly a decade in progressive leadership roles at MJHS—Fanelli plays a pivotal role in developing the social work and nursing teams. She leads quality initiatives that strengthen communication within interdisciplinary teams and during family meetings—improving the patient, caregiver and staff experience.

Fanelli is committed to equitable, inclusive care for both patients and employees. She has championed team building efforts to address regulatory and staffing challenges and introduced weekend social work visits to improve end of life quality measures. Her unique combination of clinical nursing expertise and social work insight has further shaped our compassionate, whole person model of care. She is an inspiration to colleagues and is particularly invested in mentoring clinicians new to hospice and palliative care, helping elevate quality and patient outcomes.

Another of Fanelli’s significant achievements is leading the implementation of a suicide assessment process to ensure patient safety from admission through the entire hospice journey. She trained all staff in the assessment tool, strengthening clinical monitoring and enabling thorough evaluations that support safe, informed transitions from hospital to home or facility settings. Her efforts improved assessment, planning, communication and education across all teams and partners.

Fanelli models exceptional collaboration and communication, creating an environment where individuals feel respected, supported and valued. For these all of the above reasons and more, she deservesUHFsrecognition.
